Background: The round window membrane (RWM) acts like a barrier for topically applied substances into the middle ear preventing diffusion into the perilymphatic fluid compartment. Material and Method: In an animal model, modulation of the permeability of the RWM was attempted using Streptolysin-O (SLO) in various concentrations and sodium hyaluronate. Thereafter, the effect of intratympanically applied Lidocain 2% on hearing threshold measured by auditory brainstem response was tested for Lidocain 2 % and Lidocain 2 % in combination with SLO or sodium hyaluronate. Why Do Forward Maskers Affect Auditory Intensity Discrimination? Evidence from "Molecular Psychophysics"

Nonsimultaneous maskers can strongly impair performance in an auditory intensity discrimination task. Using methods of molecular psychophysics, we quantified the extent to which (1) a masker-induced impairment of the representation of target intensity (i.e., increase in internal noise) and (2) a systematic influence of the masker intensities on the decision variable contribute to these effects. A method for determining precise electrical hearing thresholds in cochlear implant users

A psychoacoustic procedure designed for the precise assessment of perceptional threshold (T-level) in cochlear implant (CI) users is presented. Binaural release from masking in forward-masked intensity discrimination: Evidence for effects of selective attention

In a forward-masked intensity discrimination task, we manipulated the perceived lateralization of the masker via variation of the interaural time difference (ITD). The maskers and targets were 500 Hz pure tones with a duration of 30 ms. Standards of 30 and 60 dB SPL were combined with 60 or 90 dB SPL maskers. As expected, the presentation of a forward masker perceived as lateralized to the other side of the head as the target resulted in a significantly smaller elevation of the intensity difference limen than a masker lateralized ipsilaterally. Sequential grouping modulates the effect of non-simultaneous masking on auditory intensity resolution.

The presence of non-simultaneous maskers can result in strong impairment in auditory intensity resolution relative to a condition without maskers, and causes a complex pattern of effects that is difficult to explain on the basis of peripheral processing. We suggest that the failure of selective attention to the target tones is a useful framework for understanding these effects. Cochlear origin of early hearing loss in vestibular schwannoma.

Objective: To test whether early hearing loss (HL) is cochlear in origin in patients with vestibular schwannoma (VS). Study Design: Retrospective case review in an academic tertiary referral center. Methods: A group of 19 VS patients with normal/symmetrical hearing and a group of 20 VS patients with mild HL (threshold at any tested frequency better than 45 dB HL) on the tumor ear side. Differences of the amplitudes of the distortion products of otoacoustic emissions (DPOAEs) between the tumor ear and the nontumor ear were studied at frequencies of 1, 1.4, 2, 2.8, and 4 kHz. Factors Influencing Tinnitus Loudness and Annoyance

Objective To evaluate the 2 major components of tinnitus severity, loudness and annoyance, and their degree of dependence on characteristics of tinnitus manifestation, history, and etiology. Design Cross-sectional survey performed during the first months of 2004. Setting Nonclinical population. Participants A total of 4995 members of the German Tinnitus League. Intratympanic gentamicin treatment after endolymphatic sac surgery.

Interval treatment with up to three intratympanic gentamicin injections once weekly effectively controlled vertigo while preserving hearing in patients with Ménière's disease and recurrent or resistant vertigo after saccotomy.Recurrent or resistant incapacitating vertigo may occur after endolymphatic sac surgery (saccotomy) in patients with Ménière's disease. In these patients, revision saccotomy, vestibular nerve section or labyrinthectomy are the established treatment options.
